We offer Commissioner of Oath services at the County Administration Building in Cayuga. Services requiring a Notary Public are not available through Haldimand County.

How to get a document commissioned
Commissioning services are available in person during regular business hours, Monday to Friday, from 8:30 a.m. to 4:30 p.m.
The person(s) swearing the affidavit must appear before the commissioner. Any person signing the document must be present and provide government-issued identification that includes their photo and signature (such as a driver’s licence, provincial photo card, or passport).
All documents to be commissioned must be signed in the presence of the Commissioner of Oaths.
Accepted documents
Common documents we can commission include:
- adult name change documents
- affidavit for lost/stolen or damaged passport
- affidavit from the Ontario Rental Housing Tribunal
- affidavit of a lost cheque
- affidavit of proof of loss of motor vehicle/boat (from an insurance company)
- affidavit of proof of residency
- affidavit of unregistered vehicles (boats, motorcycles, snowmobiles, trailers, etc.)
- Canadian citizenship forms
- delayed statement of live birth
- domestic and foreign pension documents (Proof of Life)
- Haldimand County generated documents
- immunization health form/exemptions
- statutory declaration for motor vehicle deals compensation fund
- statutory declaration for OSAP – statement of common law status/supporting children
- statutory declaration of apprenticeship hours
- statutory declaration of lost identification (must include police report number)
- statutory declaration of theft of a GST/HST or federal cheque
- sworn statement for the transfer of a used motor vehicle in the province of Ontario
- travel affidavit
What we can't commission
Documents we cannot commission include:
- age of majority or BYID card applications
- certified true copies of documents
- court, legal or civil issue-related documents
- custody documents
- declaration of replacement of shares
- declaration of a translator
- declaration or claimant and indemnity
- divorce, separation, marriage or cohabitation
- documents not in English or that do not have English descriptors
- documents related to debt
- documents requiring a notary public’s signature
- power of attorney
- real estate-related documents
- statement of arrears
- statutory declaration of progress payment distribution – holdback security deposits
- tender documents
- wills, living wills, codicils to wills, or estate settlement
