Position Summary
This role is a fundamental part of the interdisciplinary care team that actively contributes to the well-being of residents. The incumbent will follow all Ministry regulations, under the Fixing Long Term Care Act (FLTCA, 2021) including observing residents, promptly reporting any changes, and maintaining timely and accurate documentation. The Restorative Care Personal Support Worker (PSW) will provide assistance to residents in their daily activities in the restorative care program, always upholding the Residents’ Bill of Rights, and tailoring all interactions to their care plans.
Qualifications, Knowledge, and Skills
Education
- High School Diploma or equivalent
- Personal Support Worker Certificate or deemed equivalent according to FLTCA, 2021
Experience
- Three (3) to six (6) months current related experience
- Restorative Care Certificate (or willingness to complete within 3 months of hire)

- Ability to recognize any change in condition in the Residents’ usual patterns

- Technology Aptitude:
- Demonstrated basic computer skills to perform basic tasks on various technological devices (i.e., tablets) and programs (i.e., PointOfCare; POC) while adhering to cybersecurity and an awareness of best practices

Responsibilities
The incumbent is responsible for, but not limited to:
Corporate
- Follow the GVL philosophy of delivering care to Residents by utilizing Dementiability and GPA principles.
- Ensure Residents’ Bill of Rights are upheld.
- Work within policies and procedures determined by Haldimand County, GVL and current legislation.
People
- Assist Restorative Care Lead with implementation of the restorative care program in the Home.
- Communicate with Residents, visitors, and other personnel with a customer service focus.
- Participate in admission process and family conferences.
- Participate on various teams and committees.
- Provide work direction, instruction, and training, and education to peer group and interdisciplinary team.
- Undertake continual collaboration and communication with the Director of Nursing, Associate Director of Nursing, and Nurses in the Home.
Functional
- Resident Care
- Assist Residents in the restorative care program with activities of daily living according to their personalized plan of care.
- Assess residents who have been referred to the restorative care program.
- Contribute to the completion of policy reviews and annual program evaluations.
- Sets goals, creates action plans and ensures follow up is completed.
- Assist primary PSW staff in contingency situations.
- Recordkeeping and Reports
- Participate as a member of the interdisciplinary care team through Resident observation, prompt reporting, accurate documentation and by using techniques and therapies approved for the wellbeing of the Residents.
- Completes all required reports and statistics as required for the Ministry, QIP, etc.
- Creates, analyses, and presents detailed reports on quality indicators.
- Understands how the interRAI assessment integrates with existing clinical tools and provides leadership in maintaining these processes.
- Adheres to responsibilities and requirements for reporting adverse resident occurrences subject to regulatory and legislative direction.
- Health & Safety
- Maintain any equipment used in accordance with established policies and procedures.
- Adheres to infection prevention and control practices as outlined in the policy and procedure manual.
- Ensure the Resident’s comfort and safety through compliance with current health and safety regulations and Ministry of Long Term Care standards.

Position Requirement(s)
Vulnerable Sector Screen Police Check (OPP LE 220 or Equivalent) within Past 6 Months
Two- Step TB test (Mantoux) or chest x-ray within Past 6 Months
