Position Summary
At Grandview Lodge, the InterRAI Lead plays a key role in ensuring residents’ safety, as well as promoting their physical, social, and recreational well-being and overall quality of life. As a team leader, the InterRAI Lead is responsible for implementing, overseeing, and managing the InterRAI program and its associated processes to ensure compliance with legislative and operational standards. Additionally, this role includes educating and supporting staff in the effective use and understanding of InterRAI tools to enhance care planning and delivery.
Qualifications, Knowledge, and Skills
Education
- Current regulated or health care practitioner (e.g. RPN, RN, BSW, MSW, Dietitian)
- Recognized RAI credentials or equivalent
- InterRAI LTCF certification
- Adult Education Certificate is considered an asset
Experience
- At least three years clinical experience with at least 1 year project leadership, and/or Quality Improvement Plans (QIPs) (preferably in a Long Term Care setting)
- Preferred experience with RAI/MDS coordinator role and CIHI
Knowledge/Skills
- Knowledge and experience with adult education and facilitation
- Demonstrated leadership skills, providing advice, direction and training related to the InterRAI program
- Proven ability to meet deadlines
- Proven time management skills
- Aptitude for detail orientated work
- Ability to recognize and respond to any change in resident condition
- Excellent verbal and written communication skills with the ability to influence, motivate, provide training, using flexible communication styles and to ensure suitable agreement, decisions are reached.
- Interpersonal skills to interact with residents, families, co-workers, and the general public
- Ability to work within an interdisciplinary team
- Ability to liaise with team members to coordinate assessment schedules
- Demonstrated success leading new initiatives
- Ability and willingness to attend workshops, seminars, and conferences
- Technology Aptitude:
- Demonstrated proficient computer skills to perform tasks on various technological devices and programs (i.e., PCC, POC, CIHI) while adhering to cybersecurity and an awareness of best practices
- Demonstrated knowledge with computers, printers, internet, apps, and e-mail

Responsibilities
The incumbent is responsible for, but not limited to:
Corporate
- Follow the GVL philosophy of delivering care to residents by utilizing Dementiability and GPA principles.
- Ensure Residents’ Bill of Rights are upheld.
- Work within policies and procedures determined by Haldimand County, GVL, and current relevant legislation.
People
- Undertake continual collaboration and communication with the Director of Nursing , Associate Director of Nursing, Nurse Practitioner and Charge Nurses in the Home.
- Participate in assessing, planning, implementing, and evaluating nursing and restorative care through prompt communication and documentation, observation, and use of approved techniques/therapies to contribute to the development and modification of care plans.
- Provide work direction, work supervision, work assignments, instruction, and education to front-line staff.
- Participate/lead appropriate teams and committees (i.e. Falls, Restorative care).
- Co-ordinate assignments for completion of RAI/MDS assessments.
- Act as educational resource for the interRAI program.
- Train staff on computer applications and software for RAI and MDS.
- Provide ongoing teaching and coaching of staff with the interRAI system.
Timeline
- Send electronic documentation for submission following reporting deadlines (i.e., CIHI, interRAI, MLTC).
Functional
- interRAI Processes
- Plan, organize, and oversee the completion of RAI/MDS assessments for all residents within established timelines.
- Ensure MDS assessments, resident assessments, and plan of care reviews are completed in accordance with CIHI guidelines and MLTC legislation.
- Generate required and relevant reports to facilitate in clinical and operational decision making.
- Regularly communicate with the leadership team and all RAI users across all shifts.
- Participate in orientation of new RN’s, RPN’s, PSW’s and other staff in the interRAI program.
- Other duties as assigned.
- Program Monitoring and Maintenance
- Maintain and routinely evaluate the current data submission processes.
- Assist RAI users to correctly code MDS assessments and provides support for validation of coding.
- Ensure resident assessment schedules are developed, completed, and accessible to all RAI users.
- Monitor and coach all RAI users across all shifts.
- Co-ordinate processes that ensure data accuracy, efficient data collection, assessment completion, and data utilization.
- Complete and analyze monthly/quarterly/yearly audits and indicators; identifying and creating corrective actions.
- Complete POC documentation audits and provide education to staff on POC and PCC applications across all shifts.
- Health & Safety
- Maintain any equipment used in accordance with established policies and procedures.
- Adheres to infection prevention and control practices as outlined in the policy and procedure manual.
- Ensure the Residents’ comfort and safety through compliance with current health and safety regulations and Ministry of Long Term Care standards.
- Report any and all abuse immediately to the Charge Nurse in the Home, Director of Nursing, and Administrator.
- Complete accident/incident report forms, as well as Critical Incidents.

Position Requirement(s)
- A current police check (LE 220 or Equivalent) from within the last 6 months
- Two-Step and Mantoux Test within the past 6 months
Working Conditions
- Core hours are Monday – Friday, 8:30am – 4:30pm
